Methods of alcoholism treatment
Alcoholism (dipsomania, chronic alcohololism, chronic alcoholic intoxication, alcoholic narcomania, ethylism) is progressive disease characterized by pathologic inclination for alcoholic drinks, development of alcohol withdrawal syndrome(abstinence syndrome) when alcohol is not intaken, and in some hard cases steady somatoneural disorders and psychic degradation.
Treatment period can be divided into three stages, changing one after another:
- I initial (neurasthetic) stage;
- II middle (narcotic) stage;
- III outcoming (encephalopatic) stage.
At the first stage it is obsessive mental inclination for alcohol when the quantity of intaken alcohol is unverifiable. At the second stage a new kind of disorders appears it is physical alcohol dependence with irresistible addiction to alcoholic drinks, regardless of will, reason, senses, feelings. A complex of somatovegetative abstinent disorders and internal pathological changes appear. Alcoholic psychosis can develop. At the third stage addiction to alcohol acquires compulsive dominant character. Abstinence syndrome becomes harder. Situation control is lost. Alcoholic dementia is developed. Somatic complications become harder. Very often alcoholic psychosis appear.

Out-patient treatment of alcoholism begins at the moment the patient comes to the doctor for diagnostics and care. The doctor carefully examines the patient, analyses the reasons that led to alcohol addiction and after that the doctor works out individual treatment plan, prescribes specific medicine, carries out psychotherapy, gives recommendations to the patient and his relatives.
 
The following treatment consists of three stages:
I stage treatment of alcohol intoxication. Spread before opinion that ethanol intoxication is state of comparatively good quality, has been revised nowadays. Clinical presentations of alcohol intoxication are hard to distinguish from symptoms of overdose by other CNS suppressors (benzodiazepine tranquilizers, barbiturates, and others). Typical signs of alcohol intoxication are deep tendon reflex depression, babbled voice, staggering gait; in some hard cases CNS suppression can develop into sopor and coma.
II stage treatment of alcohol withdrawal syndrome.
Alcohol withdrawal syndrome can be regarded as state opposite to alcohol intoxication in some of its clinical presentations and required medical care.
Development of alcohol withdrawal syndrome should be connected with three main factors:
- ethanol deprivation with ethanol tolerant people;
- intoxication by products of ethanol oxidative metabolism;
- metabolic disorders accompanied by chronic alcoholization, and disorders caused by malnutrition of the patients (during hard drinking people eat practically nothing).
Antiabstinent therapy carried out in the centre includes two main components:
- treatment aimed to relieve symptoms of ethanol withdrawal;
- proper detoxication, the aim of which is normalization of homeostasis.
III stage preventive (antirelapse) therapy.
